Wisconsin Hospital First to Add PVI to ACE Accreditations for Peripheral Coronary Intervention and Cardiac Catheterization

Accreditation for Cardiovascular Excellence (ACE) has awarded the world’s first accreditation for percutaneous vascular intervention (PVI) to Aurora St. Luke’s Medical Center in Milwaukee, WI. The distinction marks three ACE accreditations for Aurora St. Luke’s, which was also accredited for percutaneous coronary intervention (PCI) and cardiac catheterization (CC). Another ACE accreditation will be finalized soon as Aurora St. Luke’s is also working toward a carotid artery stenting (CAS) accreditation.

In achieving ACE accreditation for PVI, Aurora St. Luke’s, which is part of the Aurora Health Care system, has demonstrated that the hospital meets the highest standards for treating patients with peripheral vascular disease (PVD), which can be a difficult challenge for cardiovascular interventionalists. Patients with PVD tend to have long, complex lesions in addition to diabetes, hypertension, hypercholesterolemia, and other co-morbidities that frequently underlie their vascular disease.

“Given our volumes and experience in PVI, we felt ACE accreditation was the step we needed to maintain and ensure the best possible delivery of care in this subset of patients,” said Aaron Novotney, Senior Director, Integrated Growth in the Enterprise Business Group at Aurora Health Care. “Recognition in this area is consistent with our strong historical reputation for being first-in-market, first-in-region, and first in the nation for various aspects of innovation and delivery of quality cardiovascular care. Now we can say we’re the first in the world to be accredited for PVI.”

Aurora Health Care clearly views ACE accreditation for PVI as an important credential in providing quality care. “As the health care industry progresses into a data-driven, best-practices environment, hospitals must demonstrate that they deliver good quality and positive outcomes, with high levels of patient satisfaction,” commented Cindy S. The Pulmonary Hypertension Association (PHA) has announced two newly accredited Centers of Comprehensive Care (CCC) as part of the Pulmonary Hypertension Care Centers (PHCC) initiative. Programs at Vanderbilt and Aurora St. Luke’s Medical Center join the initiative in care of “the other high blood pressure.”

The Pulmonary Hypertension Association (PHA) has announced two newly accredited Centers of Comprehensive Care (CCC) as part of the Pulmonary Hypertension Care Centers (PHCC) initiative. Now found in 23 states in the contiguous United States, increased numbers of CCCs provide more options for adult and pediatric patients with pulmonary hypertension (PH).

PH is increased pressure in the blood vessels of the lungs that can lead to death from right heart failure. Common symptoms of PH can include shortness of breath, fatigue and chest pain. The disease is often misdiagnosed and treated as another illness, such as asthma or COPD. Delays in diagnosis and treatment cost patients valuable time. Because patients go months, sometimes years, believing they have something other than PH, the average length of survival for the progressive disease is only 2.8 years after diagnosis. However, the good news is that with early and accurate diagnosis, PH treatments extend and improve the quality of life for many patients.

The PHCC initiative is designed to create an accreditation program to raise the level of care for patients living with PH. Criteria are based on key measures of care for PH patients, including the program’s overall commitment to PH patients, scope of services provided and expertise of care team. The two newly accredited CCCs are:

1.    Vanderbilt Pulmonary Vascular Center – Nashville, Tenn.
2.    Pulmonary Hypertension Clinic at Aurora St. Luke’s Medical Center – Milwaukee, Wis.

The PHCC program helps to advance PHA’s mission of extending and improving the lives of people living with PH. Anticipated outcomes of the PHCC initiative include better informed patients and medical professionals, improved access to Centers with expertise in PH, and increased opportunities for research collaboration between Centers.

PHA’s Scientific Leadership Council (SLC) and the PHCC Oversight Committee set the criteria for PHCC accreditation. The SLC is composed of global leaders in the field of pulmonary hypertension including healthcare professionals and research scientists in medical centers recognized for excellence in care for patients with PH.

Since the launch of the PHA-accredited PH Care Center (PHCC) program, a total of 34 Center of Comprehensive Care (CCC) programs have been accredited, including 30 adult centers and four pediatric centers.

As Centers are accredited, they also agree to contribute to the Pulmonary Hypertension Association Registry (PHAR). This patient registry will collect data used to evaluate outcomes for people living with PH and help researchers learn more about the rare disease, leading to the possibility for new breakthroughs in treatment and quality of care.

The list is based on a comparison of the 150 largest cities across 13 key metrics, such as sales per agent and the annual median wage of agents. Denver also had the fourth-highest housing market health index.

Worcester Polytechnic Institute Professor Receives Grant to Assist U.S. Air Force in Unmanned Aerial Vehicle Research

Intelligent algorithms placed on board UAVs could assist remote military and civilian pilots and lead to improved operational safety

A Worcester Polytechnic Institute (WPI) professor has received an 18-month, $185,000 grant from the U.S. Air Force for research exploring how unmanned aerial vehicles (UAVs) can be operated safely, reliably, and intelligently even when they experience mechanical problems or encounter other unexpected situations.

Raghvendra V. Cowlagi, assistant professor of mechanical engineering and aerospace engineering, received the grant in collaboration with Aurora Flight Sciences Corp., a leader in the manufacturing and design of optionally piloted and unmanned aircraft.

The research can be applied in UAVs used in military and civilian applications, including those designed for search-and-rescue missions.

UAVs are designed to fly under the control of a human pilot on the ground. However, a loss of communications and/or onboard contingencies, such as engine problems or undetected structural damage to the airframe, can impair the ability of a UAV to complete its mission. UAVs operated by the Air Force include the General Atomics MQ-1 Predator, which has a wingspan about one-quarter the size of a Boeing 747 and can carry about 450 pounds of payload.

Even when communications are not interrupted, the large volumes of data transmitted by UAVs operated by the Air Force— including aircraft health, positioning, weather, and other factors—can overwhelm pilots during emergencies, making it difficult to assess the true nature of the situation.

“This could lead to accidents or the aircraft may not be able to operate in an optimal way,” Cowlagi said.

To address the challenge, Cowlagi and his research team are developing algorithms to run on a UAV’s onboard computer that will allow the aircraft to perform high-level missions with minimal human supervision or remote piloting, while also enabling the vehicle to tolerate severe degradation to the airframe and engines. The algorithms will also help remote pilots parse the vast amount of information transmitted by UAVs to gain a better understanding of the vehicle’s condition.

“We’re trying to determine what types of intelligent algorithms we can put on board the aircraft that can deal with these sensor inputs,” said Cowlagi. “We’d like the UAV to be able to do a lot of the computations on board and take care of these contingencies before communicating back to the pilot.”

Cowlagi stressed that the remote pilot will still be the supervisor, but the algorithms will take much of the burden of real-time decision making off of pilots. “A lot of the decision making that might have gone on in a pilot’s head will now go on board the UAV, and we think this will lead to safer operation of UAVs in the future,” he said.

In building the algorithms, Cowlagi said his team is using a novel method based on linear temporal logic (LTL) specifications. LTL is a logic system that allows the human supervisor to command the UAV, and impose safety behaviors on the UAV, all in a unified human-friendly format. The challenge, which is being addressed by Cowlagi’s research, is to design algorithms that can ensure that the UAV diligently follows these commands from the human supervisors, while accounting for health degradations in the airframe structure and/or engines.

“This research has never been used before in the context of unmanned aerial vehicles,” said Cowlagi. “We are the first group to try this and to try to apply this method in the hopes of making UAVs safe and reliable.”

Aurora leaders are pleased with the partnership with WPI.

“The collaboration between Aurora and WPI is producing exciting results, proving that self-aware and self-reacting UAVs are obtainable in the not-too-distant future,” said Jeffrey Chambers, composite structures research engineer and project lead at Aurora. “Aurora provides expertise in UAVs and helps formulate the research statement, while WPI provides novel concepts to address the operational obstacles at hand.”

Other researchers working with Cowlagi on the project are graduate students Zetian Zhang, Benjamin Cooper, and Ruixiang Du, who are conducting doctoral research under Cowlagi’s supervision.

This grant builds on previous U.S. Air Force–funded research Cowlagi has conducted in collaboration with Aurora. In particular, the feasibility of the current research was demonstrated in early 2015 through preliminary results obtained by Cowlagi and his team.

Earlier this year, Cowlagi received a Young Investigator Award from the Air Force Office of Scientific Research for a research project focused on data-driven control for UAVs.
